Primary Care and Rural Health Impact Awards Nominations

Nurse Practitioners are at the crossroads of health care and are not often recognized. Nominate a colleague or someone who makes a difference in Primary Care or in Rural Health Care before the nominations close on September 19th (2 days away!). Nomination form below.
The past several years the Utah Office of Primary Care and Rural Health has recognized a handful of Utah’s amazing rural health workforce through their Primary Care and Rural Health Impact Awards. We are once again accepting nominations for this award. Their staff presents this award to rural healthcare heroes. The Impact Award nomination window will close on September 19th.

Impact Award recipients will be recognized at the upcoming Rural Health Association of Utah (RHAU) annual meeting (November 12-13), during National Rural Health Week (November 17-21), and will be featured on the Office of Primary Care and Rural Health (PCRH) blog.
