PALLIATIVE CARE NURSE PRACTITIONER
Mobile Medical Group • We are hiring in the following location: Salt Lake, Ogden, Springville and St George, UT


Mobile Medical Group is looking for a Palliative Care Nurse Practitioner to join our team. This is a full- time, salaried with benefits position available in the Utah cities above.


RESPONSIBILITIES
• Assists with the provision of palliative care in accordance with facility, state, and federal regulations
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
• Implements interventions to support the patient with chronic conditions to decrease recurring
hospitalizations
• Collaborates with primary care physicians/NP by making appropriate referrals, follow up calls, and coordination of care
• Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients
• Prescribes appropriate pharmacological and non-pharmacological treatment modalities
• Monitors the effectiveness of interventions
• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients


QUALIFICATIONS
1. Will consider less NP experience with RN hospice and/or home care experience
2. Minimum of 5 years OR clinical hospice and palliative care experience. 2 years’ experience in the Nurse Practitioner role preferred
3. Must work well independently and within a team
4. Strong computer skills, including Word, Excel, and PowerPoint
5. At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN)
6. Current Advanced Practice Registered Nurse (APRN) Licensure
7. Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful
8. Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position
9. Graduate of an accredited School of Nursing
10. Knowledge of clinical standards of care
11. Education and/or Experience: Completion of Nurse Practitioner accredited program
12. Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems
13. Minimum of two (2) years clinical experience, desired
14. Exceptional interpersonal skills & superb communication a must
15. At least one (1) year of experience in Hospice or Palliative Care
